Adoption Network Cleveland invites individuals who have interacted with adoption facilitators, brokers, or consultants to share their experiences via a confidential online survey. We are looking for people who worked with Ohio-based entities or were referred to such an entity out of state by an agency or entity in Ohio.

If you are (or have been) a prospective adoptive parent, expectant or birth parent, or someone who considered adoption and worked with an unlicensed facilitator, your story matters. Your feedback will help inform advocacy efforts and strengthen protections for families and expectant parents across Ohio.

All responses are confidential. Identifying information will not be shared without your permission.

Your participation will help improve adoption practices and policies, better supporting others in the future.

The survey takes approximately 10–15 minutes to complete and can be accessed here.
